Immunization Requirements for School Entry

Vaccination is one of the best ways parents can protect infants, children and teens from 16 potentially harmful diseases. Vaccine-preventable diseases can be very serious, may require hospitalization, or even be deadly - especially in infants and young children.

Hawthorne School District wants to promote and support a healthy learning environment for all of our students, staff and community.  We also want to ensure you have current information regarding immunization requirements that may impact your child. To enter school, parents must present a signed or stamped immunization medical record from a physician or clinic. Students are required to meet all state immunization requirements.

New Regulations starting July 1, 2019.  New Immunization dose requirements for school admittance (TK/K-12) will need:

Immunizations Required for 7th Grade Advancement:

Tetanus, Diphtheria, Pertussis (Tdap) —1 dose
(Whooping cough booster usually given at ages 11 and up)

Varicella (Chickenpox)— 2 doses
(Usually given at 12 months of age and 4-6 years of age)

Vacunas Requeridas en el 7.⁰ Grado:

Vacuna contra el tétanos, la difteria y la tos ferina (Tdap) —1 dosis
(El refuerzo de la vacuna contra la tos ferina generalmente se aplica a los 11 años de edad o más.)

Varicela —2 dosis
(Generalmente se aplica a los 12 meses de edad y entre los 4 y 6 años de edad)

Important Notice: Vaccination Law Update and Elimination of Personal Beliefs Exemption

Under a new law known as SB 277, beginning January 1, 2016 exemptions based on personal beliefs, including religious beliefs, will no longer be an option for the vaccines that are currently required for entry into child care or school in California.  Most families will not be affected by the new law because their children have received all required vaccinations. Personal beliefs exemptions (PBE) on file for a child already attending child care or school will remain valid until the child reaches the next immunization checkpoint at kindergarten (including transitional kindergarten) or 7th grade.

  • PBEs currently on file remain valid until the next immunization checkpoint at TK/Kindergarten or 7th grade. Additional forms do not need to be submitted.
  • Students entering into TK/Kindergarten, and 7th grade on and after July 1, 2016, will be required to submit documentation at registration verifying compliance with the State immunization requirements, unless a valid medical exemption is on file with the school district. Previous PBEs will no longer be valid for advancement into these grade spans.
  • Beginning January 1, 2021, all new medical exemptions for school and child care entry must be issued through CAIR-ME. If any of these immunizations were not given to your child because of medical reasons, please bring us a medical exemption form issued using the CAIR-Medical Exemption website by your child’s doctor (MD or DO licensed in California).
